Audit/CDE Verification 


DANB has implemented a continuing dental education verification system or Continuing Dental Education (CDE) audit.  Under this system, certificants are required to maintain documentation of their own CDE hours.

Each month, DANB certificants are randomly selected for verification of their CDE.  If you are selected for an audit, you will be asked to submit a completed CDE Recording Form and copies of documentation supporting your form entries.  Education has a two- (2) year life span for renewal of certification.  Those selected for audit will be considered certified during the audit period.

  • Make copies of the DANB CDE Recording Form.  This form is to be used to record your CDE hours.  Please make several copies of this form for future use.
  • Attach one copy of the CDE Recording Form to a 9"x12" envelope or file folder.
  • Complete the CDE Recording Form, listing date, hours earned, sponsor name and title of the course.  As you complete each CDE activity, update the form, using one line per activity.
  • Keep all supporting documentation in the envelope or file folder with a current copy of the Recertification Guidelines
  • Keep all documentation for two years from the date of the course.
