This on-line database of Oregon dental assistant licensees/credential holders will verify credentials for dental assistants who earned their Oregon dental assisting credentials in 1995 and thereafter. For verification of Oregon dental assistant credentials for those who may have earned them prior to 1995, please call 1-800-FOR-DANB, extension 451 or 443 or 450.

Oregon dental assistant licenses/credentials can be accessed on-line by Social Security Number or Oregon License/Credential Number only and not by dental assistant name. Those with a legitimate need to identify Oregon dental assistant credential holders by name only can place that request, with any additional identifying information available, in writing to DANB at 444 N. Michigan Ave., Suite 900, Chicago, IL 60611. These types of requests will be subject to administrative and legal review.
